Planning for discharge

We understand being hospitalized can be a significant interruption to your daily life, and our goal is to support you in leaving the hospital as soon as it is safely possible. That’s why discharge planning begins the day you’re admitted. This approach keeps you informed and helps reduce disruptions.

For your convenience, we will update you each day on your expected discharge date. This allows you and your loved ones to plan ahead and improves coordination of the next steps in your care. A member of your care team will discuss going home plans with you, including home care, medications, follow-up appointments and other instructions to make your transition from hospital to home as smooth as possible.

If needed, a case manager and/or social worker will help plan a safe and smooth transition to your next level of care.

Going home

Our goal is to have you ready to go home by 11 am on your day of discharge. Your care team will provide a discharge time based on your plan of care, which could be earlier or later.

Once your physician clears you for discharge and your discharge time is finalized, you can coordinate your transportation with your family or caregivers. Our team can also help arrange your transportation home (via Lyft), if needed.

Discharge Lounge

You may relax in our Discharge Lounge until your ride arrives. Located on the 4th floor of the Heart Hospital directly across from the elevators, the Discharge Lounge is a comfortable and convenient place to wait for your ride, relax in a recliner and enjoy the beautiful river view while having a snack, reading or using our free Wi-Fi.
